Text: Inscribed on one face: 1 column on fragments a and c, two columns on b.
Letters: 0.01-0.03.

Date: Augustan to first century CE

Findspot: Ptolemais: a. found before 1941, findspot unrecorded, b. and c. in the West Church in 1935.
Original location: Unknown.
Last recorded location: Tolmeita Museum.
