Text: Inscribed in two columns. on one face which is very worn.
Letters: 0.01-0.012.

Date: Probably first century CE

Findspot: Ptolemais: two fragments found in the West Church, in 1935, the third in the Street of the Monuments.
Original location: Unknown.
Last recorded location: Tolmeita Museum.
